Job Description

ESSENTIAL FUNCTIONS :                                                                                                                                                           

  • Organizes, plans and directs work assignments for nursing personnel for assigned shift.
  • Confers with other departments on an as needed basis to coordinate patient care.
  • Assists in the identification of medical, emotional and environmental needs of the patient. Assesses and evaluates all medical and psychiatric problems, initiates initial interventions and reports to physician. 
  • Obtains a complete initial assessment including medical, psychiatric and/or chemical dependency history.
  • Thoroughly follows physician orders, administering medications and treatments and schedules outside consultations.
  • Assists in the formulation of treatment plans including assessments, writing of care plans, updates of care plans, discharge planning, meetings and communications regarding patient care to therapy staff.
  • Provides health teaching to staff and patients including Infection Control Education.
  • Serves as medical representative on facility committees as requested.
  • Assists in evaluation and competency assessment of nursing staff.
  • Assists in orienting nursing personnel and nursing students to the Nursing/Medical Department.
  • Adheres to Patient Rights as outlined in the White Deer Run Policy & Procedure Manual.
  • Assists in maintaining inventory of supplies and equipment as needed. Assists in ordering medications from pharmacy.
  • Participates in ongoing inservice educational programs and attends mandatory inservice trainings as scheduled.
  • Responsible for accurate medication and money counts at the beginning and ending of assigned shifts.
  • Assists in nutritional screenings, interventions with dietician, physician and Dietary Department to ensure the patient’s dietary needs are met.
  • Assists in searches of patients and their belongings to confiscate contraband and inappropriate objects including attire.
  • Provides individual supportive counseling as well as enforcing a consistent approach to patient care by supporting therapy.
  • Assists in the collection of data for Quality Assurance monitoring.
  • Provides accurate documentation on patient charts according to White Deer Run Policy.
  • Accurately follows physician orders and instructions.
  • Immediately initiates a response to fire alarm as well as other emergency situations at the facility in accordance with the White Deer Run Policy & Procedure Manual.
  • Accurately documents unusual incidents involving staff, patients and visitors utilizing Incident Reports.
  • Assists in finding replacement nursing staff in the event of call offs.
  • Assists laboratory personnel in the morning with laboratory stops and flow of patients for venipuncture.
  • Completion of other ordered diagnostic tests such as EKG, PTF, PPD and collection of urine and sputum specimens.
  • Assists in filing of documents in medical charts including photocopying documents as needed.
  • Responsible to provide assistance in the discharge of patients including proper discharge status forms, discharge instructions, prescriptions, confiscated belongings returned, valuables returned and documents signed to verify the return of these items.
  • Assists in the completion of patient records including the completion of deficiencies.
  • Obtains needed verbal and telephone orders and documents appropriately.
  • Performs duties necessary to facilitate appropriate patient care including answering the telephone, taking and screening calls, assisting with other paperwork such as consents, transportation, treatment requests and assists with scheduling of consultations and appointments.
  • All other duties as assigned.

     OTHER FUNCTIONS :                                                           

    • May be required to arrange transportation for patients who need to leave the facility for medical/psychiatric or other emergency reasons.
    • May be required to work overtime in cases of inclement weather, call offs and/or other emergency situations.
