Job Description

UTILIZES THE NURSING PROCESS in assessing, planning, and formulating nursing interventions directed towards development of an individualized plan of care and treatment modalities

PERFORMANCE STANDARDS DEMONSTRATES proficiency in psychiatric clinical skills

A.    Conducts ongoing assessment of clients from admission to discharge

B.     Identifies expected outcomes individualized to the client to include discharge planning

C.    Prescribes interventions to attain expected outcomes

D.    Evaluates the effectiveness of interventions in relation to expected outcomes

E.     Provides individual patient teaching on all identified learning needs

F.     Facilitates in Family/Significant other education as required

G.    Performs vital signs/measurements on clients as ordered and documents same in medical record

H.    DEMONSTRATES knowledge of psychiatric disorders and appropriate nursing interventions

IMPLEMENTS NURSING INTERVENTIONS identified in the plan of care that is individualized to the patient needs. Applies appropriate interventions to progress the patient towards wellness and addresses maintenance of wellness.

 

PERFORMANCE STANDARDS

A.    FACILITATES nursing groups relevant to patient needs.

B.     INITIATES medical/psychiatric crisis intervention to stabilize and ensure the well-being of the patients’ conditions.

C.    ENCOURAGES and promotes self-care activities to support patient progress to wellness

D.    PROVIDES structure and MAINTAINS a therapeutic milieu in collaboration with the patient and other health care providers

E.     ADHERES to established Nursing practices and standards of care

F.     ADMINISTERS MEDICATIONS as prescribed/ safely.

G.    TRANSCRIBES physician’s orders accurately.

H.    RECOGNIZES signs and symptoms of adverse drug reactions.

I.       PROVIDES education on prescribed medications.

 

DEMONSTRATES professionalism, accountability, and responsibility for decision making, quality nursing care, and knowledge base of psychiatric nursing care.

B.     UPHOLDS the policies and procedures, mission, vision, and philosophy of care while maintaining an atmosphere of dignity and respect for the patients.

C.    FUNCTIONS as an advocate for patient needs/ safety.

D.    DEMONSTRATES knowledge of growth and development; COMMUNICATES with patients in a manner sensitive to their age/disability/ cultural needs; IDENTIFIES safety needs including ability to detect possible signs of abuse and neglect and report same to clinical staff; IDENTIFIES special mobility needs of patients.

E.     PARTICIPATES in performance improvement and infection control activities.

 RESPONSIBLE FOR IMPLEMENTATION of appropriate nursing practices. PERFORMANCE STANDARDSA.    REVIEWS, REVISES and UPDATES medical records as needed within facility guidelines and timeframes.

B.     TRANSCRIBES and IMPLEMENTS physicians’ orders according to facility policy.

C.    DOCUMENTS care, progress, response to intervention in medical record in an accurate, thorough, and concise manner.

D.    FOLLOWS up on client care responsibilities; COMMUNICATES incomplete assignments and current client needs to staff to assure continuity of care.

E.     PROVIDES supportive and age appropriate communication to clients and documents special needs in medical records.

F.     DEMONSTRATES knowledge and competence in identifying and applying age/disability/ culturally specific information to patient populations served which includes adolescents, geriatric, chronically mentally ill, Cajun and African American patients.

 

PERFORMS OTHER DUTIES AND SPECIAL PROJECTS as required
