Job Description

ESSENTIAL JOB FUNCTIONS

ACCOUNTABILITY

  • Provide routine shift staff meetings to ensure that staff have a clear understanding of areas of opportunity.
  • Willingness and ability to address staff and floor related issues without requiring prompts.
  • Ensure that BHT’s are with patients and not hanging out at the nurse’s station.
  • Ensure that Staff RN’s make frequent rounds and physically speak with patients to complete an accurate shift assessment.
  • Add meeting minutes to communication binder to include agenda & sign in sheet for accountability and continuity between shifts.
  • Complete performance evaluations in a timely to promote accountability and expose areas of opportunity or needed improvement.
  • Track tardiness and attendance issues and address on the spot in writing.
  • Be on time for all meetings you hold and meetings that you are expected to attend.
  • Ensure that high acuity patients are documented on q-shift to include, but not limited to (1:1’s, SAOB, SIB, aggression)

COMMUNICATION

  • Notify Director of Nursing via text or phone call regarding any major staff/patient related issue &/or incident reports.
  • Attend weekly (until otherwise determined) House Supervisor meeting to discuss current issues, areas of opportunity & POC updates
  • Complete House Supervisor Daily Report Sheet
  • Meet with each member of your team immediately to establish yourself as their leader & use this opportunity to set expectations

EXPECTATIONS

  • At the beginning of shift notify DON Census & Staffing ( # of RN’s, # of BHT’s, any 1:1’s to include name, any call-outs or no-call, no-shows. *Please include area (e.g. Sage, Cholla, etc.)
  • Order Unit Supplies weekly (designated day TBD). Place copy of order form in DON box
  • Assist on Unit as needed to increase efficiency and effectiveness
  • As a team, we will improve provider/nurse relationships and restore physician confidence in our team.
  • Treatment Team compliance: TX plans are present for any active medical problems that we are treating.  Deferred problems have justification or explanation (e.g. problem stable, no current or active signs or symptoms or patient complaints etc.)

PATIENT CARE AND SUPPORT

  • Delivers nursing care, observes, records and reports changes in patient’s condition, applies patient teaching process concerning health maintenance, prevention and rehabilitation.
  • Assesses and plans nursing care of patient from admission through discharge.
  • Updates or creates plan of care for patients as needed for medical issues and reviews at staffing’s.
  • On call and covers needed shifts as needed.
  • Effectively implements and utilizes patient care/treatment play through nursing assessment, planning, evaluation and intervention;
  • Evaluates patient’s behavior changes and choices;
  • Administers medication and follows medication policy and procedures;
  • Interprets policies and goals to patients, families and/or significant others, and to specific community groups.

PRIORITIZATION

  • Ensure that all issues that are currently being addressed in our POC are your daily priority & documented in your House Supervisor Binder
  • Face to Face Restraint Documentation
  • Fall Precautions policy is followed (yellow bands, room near nurse’s station etc.)
  • Group facilitation, participation & documentation (RN’s & BHTs)
  • Ensure staffing is appropriate per Acuity
  • Ensure that Huddles occur and are documented
  • Reduce Medication Errors (frequently monitor transcribed orders etc.)
  • Incident Report completed & notifications made.

RECORDS MANAGEMENT

  • Maintains accurate, timely documentation of notable quality, as per program and nursing requirements, in clinical and other unit records supporting a reflection of treatment plans and continuity of care.

SPECIALTY COMPETENCY

  • Demonstrates phone crisis intervention skills with intake calls.
  • Demonstrates considerable knowledge of general nursing theory and practices.
  • Demonstrates considerable knowledge of techniques, procedures, and medication administration/pharmacology specific to psychiatric care.
  • Effectively participates in the multidisciplinary health care team.
  • Identifies and fulfills education needs to enhance skills related to psychiatric nursing.
  • Demonstrates ability to use independent judgement in decision making and follow through.
  • Develops oral or written presentations for co-workers on information acquired at seminars and conferences.
  • Participates in health related community outreach activities.
  • Prepares for and actively participates in own performance appraisal and annual good setting.
  • Implements performance goals and objectives with appropriate follow-up.

TREATMENT TEAM PARTICIPATION

  • Communicates and works closely with Medical Staff and counseling personnel.
  • Participates in ongoing scheduled in-service and staff development programs.
  • Develops satisfactory, cooperative interpersonal relationships with staff members.
  • Participates with Oasis Behavioral Health’s multi-disciplinary staff in assuring and maintaining quality patient care.
  • Participates as a team member in functions and activities structured with
  • Oasis, including the staffing process.
  • Evaluates total nursing care through team conferences, nursing audit, peer review, shift reports and collaboration with other disciplines.
  • Assumes shift change responsibilities, directs and supervises nursing activities of team members; assigns and delegates nursing functions, as indicated.
  • Acts as a representative in the community for professional nursing and a team staff member for Oasis.

ASSESSMENT SKILLS

  • Uses the nursing process to perform assessments based on holistic approach.
  • Coordinates and implements specific nursing interventions based on plans of care using available resources and services.
  • Recognizes, interprets, and reports changes in client condition/diagnostic reports and follows plan of action.
  • Evaluates intervention outcomes.
  • Completes and processes all required documentation.
  • Documents assessments, interventions, and client responses to interventions in client records.
  • Documentation is legible, free of errors and omissions and uses only approved abbreviations. This includes date, time problem number, signature and status.
  • Documentation is comprehensive, objective, timely, ad concise.

TEACHING & SUPERVISION

  • Assists in development, implementation, and evaluation of teaching plans; adapts teaching methods to level of client understanding.
  • Coordinates resources and services.
  • Involves client/family in plan of care.
  • Conducts teaching in-group, individual, and family sessions.
  • Provides supervision, corrective action, and evaluation of nursing staff.
  • Creates and revises nursing schedule as needed.
  • Provides education and meetings for the nursing staff.

LEADERSHIP & TEAMWORK

  • Participates in projects to improve client care.
  • Acts as a role model for nursing practice, is aware and adheres to the standards of practice for Registered Nurses for the State of Arizona.
  • Establishes and maintains collaborative working relationships with other professionals and par-professionals.
  • Applies an organized and systematic approach in carrying out assignments and bases patient care assignments on level of care needed and ability of staff on duty.
  • Participates in group problem solving, assists others as needed.
  • Practices and suggests improvement for cost containment measures.
  • Facilitates change processes based on system review to improve quality of services provided.
  • Supervises team members as assigned.
  • Uses effective listening/communication skills and establishes channel of communication.
  • Gives and accepts constructive feedback along communication channels.
  • Assists Director in budget and staffing ratio compliance.
  • Participates in quality improvement activities including quality program development, surveys, monitors and recommendations.
  • Demonstrates good working knowledge of applicable statues, standards and regulations pertaining to mental health and nursing care.
  • Demonstrates trained competency in coordinating interventions in de-escalation of psychologically based emergencies.

QUALIFICATIONS

EDUCATION

  • Graduate of an Accredited School of Nursing.

CERTIFICATION/LICENSES

  • Licensed with State of Arizona as a Registered Nurse and in good standing with the Board of Nursing.
  • Must possess Certificate of cardiopulmonary Resuscitation.
  • Must possess valid Arizona driver’s license and ability to drive vehicle with ten-passenger capacity.
  • Must be 21 years of age and must have or be eligible to obtain an Arizona DPS fingerprint clearance.

EXPERIENCE

  • One (1) year of psychiatric or chemical dependency nursing experience is preferred.
  • May have one (1) year nursing experience in the related fields of intensive care nursing, hospice nursing, community nursing, correctional nursing or med/surg. nursing; or demonstrate a broad knowledge of nursing practices, with the ability to apply specific nursing skills to the intervention, care and progress of the behavioral health patient through the interview process.
