Job Description

  • Treats patients & families with respect at all times.

  • Provides care based on specific culture, values & any special needs of patients & their families.
  • Assesses on patients’ ongoing ability to care for self (grooming, hygiene, ambulation)Is able to identify significant changes in patients’ physical, emotional or mental status and reports these promptly to team leader.Is familiar with, observes for and reports promptly symptoms of adverse drug reactions and/or therapeutic response to medications.
  • Utilizes treatment plan in guiding patient’s review of their goals for the day in community meeting and wrap-up.
  • Attends & participates in treatment team meetings and/or provides input to RN & other team members in developing short-term goals that will facilitate patient progress.
  • Addresses patient progress in meeting short-term goals in chart documentation.Communicates promptly to nurse any significant changes.
  • Writes accurate & complete progress notes on assigned patients that address patient’s reason for admission & progress on short-term goals.
  • Checks posted deficiency lists & individual notices regularly & corrects items promptly.
  • Demonstrates familiarity with organization of hospital policy manuals & refers to & utilizes policies as guidelines for practice.
  • Demonstrates good ability to identify patients that are showing indications of increasing risk to self or others.
  • Utilizes appropriate de-escalation techniques and/or reports to team leader.
  • Demonstrates good de-escalation skills and code team participation.
  • Searches patient belongings carefully for contraband.
  • Assists in patient searches.
  • Monitors and treats detox patients safely per applicable policies and protocols.
  • Demonstrates good understanding of & practice of the principles of universal precautions through handwashing & use of barriers (e.g. gloves during codes).
  • Manages & disposes of sharps & hazardous wastes in keeping with policies & principles of Infection Control.
  • Demonstrates care for the physical environment to create & maintain an attractive, therapeutic & safe setting for work & recovery.Identifies & reports equipment or environmental problems promptly for correction.
  • Demonstrates good knowledge of hospital fire, security & disaster plans/ policies.
  • Reports incidents promptly to charge nurse and/or supervisor (at least prior to leaving building) & reviews significant incidents to identify possible causes & preventative strategies.
  • Contributes actively to searching root cause analysis of any critical incidents that occur.
  • Demonstrates competence in leading psycho-educational groups.
  • Teaches patients to use stress-reduction skills, use good interpersonal skills & accountability for self, regarding addictions & disease process/recovery skills.
  • Utilizes an understanding of group process dynamics in facilitating patient groups.
