Job Description

  • Administer medication and medical treatment in accordance with policies and physician’s orders.

  • Provides psychiatric nursing interventions and treatment education, provide intervention appropriate to the specific treatment needs of the patient.

  • Coordinate admissions, transfers, and discharges, and assure that relevant nursing documentation is completed in a timely manner.

  • Assists RN with special treatment interventions such as de-escalation, physical restraint, seclusion, nonviolent restraint, and special precautions.

  • Provides supervision and quality assurance checks to assure patient supervision, environmental safety, therapeutic structure, facilitation of patient groups and activities of daily living, and delivery of quality patient care by direct care workers.

  • Provide timely, accurate, and thorough documentation of patient behavior, medication administration, medical interventions, treatment planning, and treatment interventions. Documentation reflects professional standards.

  • Prepares and gives injections and enemas, monitors catheters, records food/fluid input/output, dresses wounds.

  • Collects samples for testing, performs routine laboratory tests as directed by supervisor, prepares and administers (orally, subcutaneously, through an iv) and records prescribed medications.

  • Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed practical nurse

  • Assists in relevant health teaching for patient and family/significant other

  • Assists CNAs with lifting and transports, using proper body mechanics or lifting devices for accident prevention.Assists with patient ambulation, transfers from bed to wheelchair; turn or reposition bedridden patients. Implements fall precautions.

  • Examine patients for bruises, blood in urine or other injuries/wounds.

  • Answer patient calls for assistance; interacts with patients, observe behaviors and communicate significant observations to medical staff.

  • Oversees or assists patients with activities of daily living, including bathing and dressing patients, toileting, grooming, oral hygiene, prosthetic appliances, transferring, ambulation, range of motion, communicating or other needs in keeping with the individuals’ care requirements.

  • Serves meals and help patients eat, provides snacks, hydration. Ensures nutritional items, meals, and fluids are consistently made available to patients in a timely manner.

  • Obtain patient's vital signs, height and weight as assigned and document in patient record.

  • Engages patients in rehabilitative groups, activities and interactions designed to encourage achievement of treatment goals.

  • Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items according to established policy and participates in efforts to locate missing items.

  • Assures that supervision is conducted at 15 minute intervals, as noted in special precautions, or in accordance with individualized supervision guidelines.

  • Communicates therapeutically and professionally with patients, visitors, case managers, coworkers, and others involved in the patient's treatment in an effective manner.

  • Completes and maintains required documentation. Assists with follow-up and paperwork as required on incidents and events that may take place in the facility.
