Job Description

  1. Assists charge nurse in data collection upon patient admission for complete patient history and assessment. Assists in orienting new patients.
  2. Collects any specimens and explains procedures to patient before and during procedures.
  3. Identifies any overt changes in patient physical, emotional, and behavioral status and report and records these to the charge nurse.
  4. Identifies specific needs from assessment, age specific behavior/needs, patient verbalization, patient history and document on plan of care.
  5. Documents progress on flow sheets and in patient progress notes and in report to nursing personnel.
  6. Understands basic pharmacology and safely administers medications as ordered according to policy and procedures within the guidelines and limits of nursing license.
  7. Advocate for patient and or family.
  8. Effectively communicates information/concerns to nursing, clinical, medical personnel and other members of the treatment team.
  9. Adheres to policies and procedures of the hospital.
  10. Assures attendance of patients in timely fashion to scheduled program activities.
  11. Maintains current licensure and practices within the guidelines/scope of the license.
  12. Assists charge nurse in patient discharge procedures.
  13. Assists charge nurse in patient education.
  14. Demonstrates consistency in work attendance.
  15. Routinely documents initiation of any antibiotics, on monthly infection control record.
  16. Reports safely issues/problems/concerns/to charge nurse.
  17. Actively involved in performance improvement/infection control.
  18. Demonstrates flexibility and willingness to help meet the hospital and/or staff needs.
  19. Serves as a resource for patients, families, and other members of the treatment team and substance abuse technicians.
  20. Acts as a peer preceptor for new orientation employees.
  21. Addresses concerns or conflict with appropriate party or director of nursing if additional guidance is needed.
  22. Willing to adapt to change – is flexible. Open to feedback from charge nurse, DON, and other management team members.
  23. Portrays in appearance, attitude and in communications as a professional image to insure/facilitate patient trust.
  24. Attends educational activities, which maintain and enhance nursing skills and knowledge in chemical dependency.
  25. Assists in initial and on-going identification of patient problems and strengths.
  26. Works cooperatively with interdisciplinary team members to assist in the development of treatment plans.
  27. Communicates patient status to registered nurse.
  28. Provides individual and group intervention to meet patient needs as directed.
  29. Leads or co-leads supportive groups, as assigned.
  30. Supports patients in development social, daily living and problem solving skills through individual, group or milieu contact.
  31. Responds to and follows through with patient needs and requests.
  32. Assists patients in their relationships with others in the milieu.
  33. Documents timely, accurate and appropriate clinical information in patient’s medical record.
  34. Assists in providing a safe, secure and comfortable environment for patients, significant others and staff.
  35. Demonstrates competency in providing care to adolescent and adult patients in inpatient mental health care settings.
  36. Visually observe and document patient’s verbal and behavioral responses.
  37. Communicate clearly and effectively to patients, families, guests and other members of the health care team.
  38. Maintains customer information in confidential manner.
  39. Attends hospital orientation, staff meetings and keeps all yearly mandatories up-to-date as required by HOH.
