The Acadia Healthcare Comprehensive Treatment Center (CTC) division, is looking for an experienced Hub Care Navigator . This is a full-time position operating within a California Opioid Use Disorder/Substance Use Disorder Grant.

  • The Care Navigator is responsible for coordinating/scheduling and assisting patients with services and ensuring follow-up is completed and providing brief supportive check-ins
  • Maintains frequent face-to-face contact and acts as a liaison for patients with the referral network
  • Meets with prospective patients at Community Based Organizations to assist with warm-handoffs to Spoke organizations as well as with other community based organizations where referrals are made.
  • Performs community outreach and education
  • Directs patients in a clear, concise, and informative manner
  • Contributes to the Comprehensive Treatment Plan through participation in treatment planning process
  • Demonstrates good moral character and even temperament necessary to gain and maintain credibility and trust of persons being served, peers, and management staff

Job Requirements

  • College degree or equivalent experience.
  • Peer experience preferred with high level of understanding of patient backgrounds, lifestyles, history.
  • Scheduling/Care Coordination experience.
  • Experience with substance abuse and/or medical care.
  • Knowledge of office administration procedures with the ability to operate most standard office equipment.
  • Ability to work professionally with sensitive data & information while maintaining confidentiality.
  • Excellent interpersonal skills including the ability to interact effectively and professionally with individuals at all levels; both internal and external.
  • Exercises sound judgment in responding to inquiries; understands when to route inquiries to next level.
  • Self-motivated with strong organizational skills and superior attention to detail.
  • Must be able to manage multiple tasks/projects simultaneously within inflexible time frames.  Ability to adapt to frequent priority changes.
  • Capable of working within established policies, procedures and practices prescribed by the organization.


Normal hours vary between 5:30 a.m. to 4:00 p.m., Monday–Friday. However, schedules may vary in accordance with business necessity. Typical office working conditions
