Overview

We are currently seeking a full-time Case Manager - Intake Coordinator for our Habit OPCO-Dunmore CTC facility. Hours are Monday - Friday 8:00 AM - 4:00 PM; evenings and weekends as needed.

Habit OPCO-Dunmore CTC, a member of Acadia Healthcare's Comprehensive Treatment Centers, is a part of the leader in medication assisted treatment for individuals struggling with an opioid addiction. With full-circle care that includes a blend of therapies and the use of safe and effective medications, we are prepared to treat the entire problem, not just a piece of it.

Position Characteristics 

The Case Manager - Intake Coordinator is a professional who completes Level of Care Assessments for new patients and manages the patient's cases with a solid background of clinical knowledge and expertise.  This Care Manager may be responsible for providing some therapeutic services to patients. Provides quality care through developing, implementing, managing and evaluating patient care plans. Collects clinical data, correlates this clinical data and provides clinical input to the treatment team. Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted. May also correlate clinical data for business office as required.

Responsibilities
  • Providing case management and care coordination services for patients in the clinic. Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;
  • May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;
  • Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team’s relationship within the facility or treatment milieu; and,
  • Perform other duties as assigned.

Job Requirements

  • Bachelor’s degree in Social Work, Counseling or related Human Service field from an accredited college or university
  • 1-2 years of experience working in behavioral health or addiction recovery setting preferred
  • DDAP core competancy trainings completed or able to complete within 6 months of hire
  • Previous experience in case management and/or utilization review preferred

Licenses/Certifications: 

  • Current and active clinical licensure/credential or active endorsement to license/credential
