Overview

We are currently seeking a full-time Care Management Support Specialist/Certified Recovery Specialist for our Dunmore CTC facility. Hours are Monday - Friday 8:00 AM - 4:00 PM, evenings and weekends as needed.

Dunmore CTC, a member of Acadia Healthcare's Comprehensive Treatment Centers, is a part of the leader in medication assisted treatment for individuals struggling with an opioid addiction. With full-circle care that includes a blend of therapies and the use of safe and effective medications, we are prepared to treat the entire problem, not just a piece of it.

Position Characteristics 

The Care Manager/CRS assists the care management team in providing a seamless continuum of care. Maintains regular and constant communication with insurance companies, finance department, treatment team members, patients and their families to increase quality and cost-effective outcomes. Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted. May also correlate clinical data for business office as required.

Responsibilities
  • Providing care management and care coordination services for patients in the clinic. Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;
  • May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;
  • Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team’s relationship within the facility or treatment milieu; and,
  • Perform other duties as assigned.

Job Requirements

  • High School Diploma required, with an Associates’s or Bachelor's degree in Social Work, Counseling or related Human Service field from an accredited college or university preferred.
  • Certified Recovery Specialist credential or working toward credentialing, if applicable.
  • 1-2 years of experience working in behavioral health or addiction recovery setting preferred.
  • Previous experience in case management and/or utilization review preferred.
