Job Description


  1. Acts as role model for staff by promoting professional practice through effective team building, communication, and advocacy.
  2. Listens respectfully and carefully, demonstrating flexibility in working with others.
  3. Demonstrates sensitivity to people of difference cultures and works effectively with them.
  4. Identifies opportunities for workflow/operational improvement in patient care planning and care delivery and presents to CNO using evidenced-based literature to support case for improvement.
  5. Demonstrates and promotes critical thinking skills and effective nursing judgment to provide therapeutic interventions to meet the patient’s needs.
  6. Ensures the Charge Nurse is effectively managing their unit by:   
  7. Manages assignment process to include delegating assignments to appropriate staff during their assigned shifts. Assignments are based on patient acuity, staff level, ability, and program schedule.
  8. Makes appropriate change to assignments as patient/unit needs/acuity changes; Communicates changes in level of care/acuity to CNO, Assessment and Referral Staff, and AOC.
  9. Hold Charge Nurse accountable to ensures that tasks are carried out as assigned in a timely fashion.
  10. Ensures on-coming staff (RNs, LPNs, and MHTs) participates in the change of shift report 7a-7p and 7p-7a to receive report on patient’s current status. All Charge Nurses will tape shift report to be available to the patients as the on-coming staff receives report.
  11. Acts as liaison between interdisciplinary team members to ensure that behavioral care plans are developed and appropriate to meet the needs of the patients. The nursing care plan is updated and appropriate as problems develop or goals and interventions are met.
  12. Will make unit rounds to troubleshoot throughput issues and resolve other issues/ concerns.
  13.  Ensure the Charge Nurse is completing incident report prior to final submission.
  14. Will utilize time on Monday and Friday to provide training and education for nursing staff.
  15. Acts as role model to staff for customer service to families and visitors.
  16. Acts as unit representative and meets with visitors/ families and answers any questions they may have.
  17. Act as first line intervention to resolve any family/visitor complaints or concerns and appropriately notifies CNO for issues that cannot be resolved.
  18. Actively engages families/visitors to promote / educate on the personal belongings campaign.
  19. Participates in unit-wide audits as indicated. Will conduct audit within 24 hours of admission to ensure time lines are being met and plans of care are developed appropriately.
  20. Ensures that restraint log is accurate and follows-up with individual staff members for completion where indicated.
  21. Ensures that established procedures are maintained for tracking and managing personal belongings and takes ownership to follow-up with nursing staff.
  22. Ensures that nursing staff are appropriately tracking and maintaining hearing aids, eyeglasses and dentures, including labeling, storage, and per documentation.
  23. Ensures Nursing Staff are documenting accurate interventions according to the plan of care during their shift rotation and will evaluate for completion and content:
  24. I&O documented as indicated
  25. graph vitals signs and weights as ordered
  26. all information required on MAR documented correctly (initials, vital signs, allergy)
  27. nursing assessment complete without any blanks
  28. allergy is documented on all physician order forms
  29. random audit of charge nurses documenting temperatures on refrigerators
  30. medication nurses documenting infections in binders on each unit
  31. Ensures MHTs are conducting thorough room checks to maintain a safe environment.
  32. Ensures that patient’s medical complaints are identified and properly treated.
  33. Follows up with responsible staff member where indicated to ensure that documentation is complete.
  34. Will participate in monthly Nursing meetings. Will also participate in training activities as indicated.
  35. Will assist CNO with evaluating the performance of staff at time of hire and on annual basis. Will hold staff accountable with meeting regulatory requirements for CMS, joint commission, and facility guidelines.
  36. Assist the Assessment and Referral Staff to ensure patients are meeting admission criteria.
  37. All Infection control reporting and rounds as assigned by the CNO.
