The Acadia Healthcare Comprehensive Treatment Center (CTC) division, is looking for an experienced Hub Case Manager. This is a full-time position operating within a California Opioid Use Disorder/Substance Use Disorder Grant.

  • The Case Manager is responsible for performing initial and ongoing assessment; coordinating and/or providing counseling services; managing acute crises; providing brief supportive
  • Maintains frequent face-to-face contact and acts as a liaison for patients with the referral network
  • Contributes to the Comprehensive Treatment Plan through participation in treatment planning meetings and care conference meetings
  • Assists patients in agreement with their counselor, to seek social, vocational and rehabilitative services on an as needed basis
  • Performs community outreach, education and in-services
  • Ability to self-motivate and possess strong time-management and organizational skills while managing a high volume of client interactions
  • Demonstrates good moral character and even temperament necessary to gain and maintains credibility and trust of persons being served, peers, and management staff

Job Requirements

  • Bachelor's Degree in Social Work, healthcare or in a human service related field required 
  • Master’s Degree in Social Work, healthcare or in a human service related preferred 
  • AOD Certification or Registration required  
  • Experience with substance abuse and/or medical care
  • Excellent interpersonal skills including the ability to interact effectively and professionally with individuals at all levels; both internal and external
  • Must be able to manage multiple tasks/projects simultaneously within inflexible time frames. Ability to adapt to frequent priority changes


Normal hours vary between 5:30 a.m. to 4:00 p.m. Monday-Friday. However, schedules may vary in accordance with business necessity. Typical office working conditions.
