Your Job as a Case Manager:

The Case Manager is a clinical professional who manages the patients' cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to patients. The Case Manager provides quality care through developing, implementing, managing and evaluating patient care plans. They will use this information to provide Targeted Case Management and connect individuals to resources needed in the community to help them achieve and maintain recovery. The Case Manager conveys medical criteria and clinical information between the insurance provider and treatment team as warranted and may also correlate clinical data for the business office as required.

We have 8 hour, daily shifts that allow our employees to have stable schedules and enjoy quality time away from the facility. Operating hours are Monday through Friday 6:00 AM to 2:30 PM, Saturday and Sunday 7:00 AM to 9:00 AM.

Responsibilities

  • Targeted case management servies are furnished to assist a patient in gaining access to needed medical, social, educational, or other needed services and supports including:
    • Assessment of the patient's medical, social, and functional status and identification of the patient's strengths and needs;
    • Arranging for service delivery from the patient's chosen provider to insure access to required services;
    • Facilitating access to needed services by explaining the need and importance of services in relation to the patient's condition;
    • Facilitating access, quality, and delivery of necessary services; and
    • Preparation and maintenance of case record documentation to include care plans, forms, reports, and narratives as appropriate.

Job Requirements

Your Skills and Qualifications as a Case Manager:

  • Bachelor’s degree in a behavioral science field.
  • 1-2 years of experience working in behavioral health setting providing case management services. Experience with the population served by the clinic preferred.
  • Previous experience in case management preferred.

Licenses/Certifications: 

  • TCADC required. CADC or LCADC preferred. Must complete Targeted Case Management training within 6 months of hire if not previously completed.
